Basement sewer pipe thru wall leaks.

We have occasional water leaking around the PVC sewer pipe that goes through the basement concrete wall to the outside septic system. How should we seal around this pipe....and should we seal it on the outside by digging down to expose it? What material would you suggest to seal around it?
